Treatments tried are listed under a wound only when it does not meet that expected %. If the wound was present less than 4 weeks, the 4-week amount is divided by 4 for each week. If a wound resolved and a later note says Progress: reoccur, or Encounter: Initial again, the row starts at that latest visit and the initial size says reoccur. Wks with a + means this pack is missing Encounter: Initial, or that reoccur visit.

Healing rate is the % change in wound area from the first to last measurement. A distinct wound is the same patient and wound number as Tracking summary. If it resolved and later the note says Progress: reoccur, or Encounter: Initial again, the row is that latest course and Wks says reoccur. A resolved/closed wound is 100% improvement. Wounds with no size, or only one dated visit, are listed as insufficient data. Wks is calendar weeks for that course; a + means Encounter: Initial, or that reoccur visit, is not in the pack. Ranked best-to-worst. Patient names stay on the report.

Pressure ulcers are grouped by stage (1-4, Unstageable, or Unidentified if the line under the wound number is blank). Acquired is the right side of the Encounter line (Facility or Community; blank is -). Wks is the current course, and a + means Encounter: Initial, or the Progress: reoccur visit, is not in the notes. Resolved lists when it resolved and, if it came back, when it reoccurred. Patient names stay on the report. Deep tissue injury is listed in a note and is not counted as a staged pressure ulcer.

Each month lists only the wound titles that appear in the notes. A title with none that month is left off. Each time a wound resolves and later says Progress: reoccur, or Encounter: Initial again, is another line. First noted for that line is the reoccur date and says reoccurred. Pressure ulcers stay split into facility acquired and community acquired when the note says so. Healed means that course resolved or closed in that month. Patient names stay on the report.

This report counts how often patients develop a new wound. An Initial Progress Note, the line above the patient name, is the baseline. A bar graph shows each pressure ulcer that starts with an Initial encounter or a reoccur. One already present on the oldest notes is left off. The bars split Initial, Initial on a patient who already had a pressure ulcer, and reoccur, with a line for the weekly total. For each patient who had a pressure ulcer, the report shows how soon a different pressure ulcer showed up and where it was. The wound list is at the end. Pressure ulcer, MASD, and incontinence-associated dermatitis (IAD) are highlighted, including when (IAD) wraps onto the next line. Patient names stay on the report.

End of week census Unit length

Census can be left blank. The log keeps only each resident's most recent visit, and only pressure ulcers and deep tissue injuries. Unit length is how many characters at the start of the room name the unit, the part of the building that room is in. Room M142W with 2 is unit M1, and the room is still written in full. Leave unit length blank to leave units off. Whole facility is one report. When a unit length is set, worsened, change in stage or type, and current injuries each list every unit under that heading. Each unit gives every unit its own report in the same PDF. Rows are sorted by room. An initial encounter is highlighted.

Unit length

Every wound except pressure ulcers and deep tissue injuries. Only the most recent visit in the batch is listed. A patient with no note on that date is left out, unless their latest note is the visit immediately before it. Those patients are listed under Not seen and are not in the current list. Unit length can be left blank. Status is written out.
