Healing rate is the % change in wound area from the first to last measurement. A later resolved/closed wound is 100% improvement. Wounds with no size, or only one dated visit, are listed as insufficient data. Acquired (before Wks) is Facility, Community, or - from the Encounter line. The summary also shows average and median reduction per week, wounds closed, and average area change per week. Wks is calendar weeks from the first to last dated note; a + means this pack has no Encounter: Initial for that wound. Ranked best-to-worst. Patient names stay on the report.

Pressure ulcers are grouped by stage (1-4, Unstageable, or Unidentified if the line under the wound number is blank). Acquired is the right side of the Encounter line (Facility or Community; blank is -). Patient names stay on the report. Deep tissue injury is listed in a note and is not counted as a staged pressure ulcer.

Monthly report card

Each month lists only the wound titles that appear in the notes. A title with none that month is left off. Pressure ulcers stay split into facility acquired and community acquired when the note says so. Every other title is listed as it is charted. Healed means the note charts that wound resolved or closed in that month. The wounds for each title are listed under the overview. Patient names stay on the report.

Treatment change check

Drop a batch of notes. The check runs on this computer from the same Patient, Date of service, Wound(s), and Treatment Recommendations fields as the other reports. Notes are not uploaded.

Two rules. After a wound is marked stalling or stalled for 14 days, the treatment must change. Each later visit still stalling on that same treatment counts again. When a wound is marked deteriorating, the treatment must change from the previous visit. The report counts how many times each rule is followed and how many times it is not. Patient names stay on the report.
